NOTICE IS HEREBY GIVEN that on November 03, 2011, the Board of Medicine, received a petition for waiver or variance filed by Richard A. Amato, M.D., from Rule 64B8-9.0131, F.A.C., with regard to the requirement for completion of 15 hours of CME in pain management for every year the physician is practicing pain management.
