RULE NO.: RULE TITLE:
64B17-3.001: Licensure as a Physical Therapist by Examination

Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 38 No. 5, February 3, 2012 issue of the Florida Administrative Register.

The change is in response to written comments submitted by the staff of  the Joint Administrative Procedures Committee. The changes are as follows:

1.  Form DH-MQA 1142 will have a revision date of 08/12.

2.  The former g. through h. are being renumbered as f. through g. There will be no change.

3.  456.013, 486.041, 486.061, F.S. will be added to the Law Implemented.

4.  In regards to the incorporated form, the $25 law exam processing fee has been removed from the application.

5.  Typographical errors have been corrected in the application.
