PURPOSE AND EFFECT: This rule sets forth two forms to be used by the Agency for Workforce Innovation in the administration of its Unemployment Compensation Program. AWI Form AWA-01 (Rev. 3/2005) is used to process request for confidential unemployment compensation records and AWI form ERWC (New 02/06) implements the statutory provisions regarding release of confidential unemployment compensation information in workers’ compensation cases set forth in Sections 443.171(5), 443.1715(1), and 443.1715(2)(b)1., Florida Statutes.
SUMMARY: Rule 60BB-3.029 sets forth forms to be used by the Agency for Workforce Innovation to aid in the administration of its Unemployment Compensation Program. Any person who wishes to provide information regarding a statement of estimated regulatory costs, or provide a proposal for a lower cost regulatory alternative must do so in writing within 21 days of this notice.
