NOTICE IS HEREBY GIVEN that on April 25, 2006 The Board of Physical Therapy Practice, received a petition for Andrea Kalkman, an amended petition seeking a waiver of the requirements of Rule 64B17-4.002(2), (3), Florida Administrative Code. The Petitioner seeks a waiver of the requirement to retake the National Physical Therapy Examination for Physical Therapist Assistants.
