(Severance Tax and Fees-Computation of Phosphate Rock Tax Rate). Approval of the proposed amendments to Rule 12A-1.097, F.A.C. (Sales and Use Tax-Public Use Forms), Rule 12A-16.008, F.A.C. (Rental Car Surcharge-Public Use Forms), Rule Chapter 12A-19, F.A.C. (Communications Services Tax), Rule 12B-4.003, F.A.C. (Documentary Stamp Tax-Public Use Forms), Rule 12B-5.150, F.A.C.
