To obtain a optometric faculty certificate pursuant to Section 463.007, F.S., the applicant must submit an Initial Optometry Faculty Certificate Application, form number DH-MQA 1134, 8/08 and the fees set forth in Rule 64B13-6.001, F.A.C.
