PURPOSE AND EFFECT: The Board proposes the rule amendment to modify and adopt the application for examination and to delete language in the subsection regarding application after revocation of license.

Any person who wishes to provide information regarding a statement of estimated regulatory costs, or provide a proposal for a lower cost regulatory alternative must do so in writing within 21 days of this notice.

64B12-9.0015 Application for Examination and Licensure.
(1) Any person seeking licensure as an optician shall submit to the Board a completed application to take the examination on Form DH-MQA 1065, Application for Licensure Examination, (revised 11/08), hereby adopted and incorporated by reference, which can be obtained from the Board of Opticianry’s website at www.doh.state.fl.us/mqa/opticianry 3/02 provided by the Department. The application shall be accompanied with the application fee specified in Rule 64B12-11.002, F.A.C., which is non-refundable, and the examination fee specified in Rule 64B-1.016, F.A.C., which shall be refunded if the applicant is denied examination or does not timely complete application.

(7) A licensee whose license has been revoked may reapply for licensure. Such applicant shall satisfy all current requirements and qualifications required by an initial applicant including examination and shall pay all outstanding administrative fines and penalties before reapplying for licensure. The applicant shall follow the same application procedures as initial applicants for licensure.
