(1) This rule applies to all Familial Dysautonomia waiver services providers enrolled in the Medicaid program.
(2) All Familial Dysautonomia waiver services providers enrolled in the Medicaid program must be in compliance with the Familial Dysautonomia Waiver Services Procedure Codes and Fee Schedule, July 2009, which is incorporated by reference.
