REQUEST FOR PROPOSALS
Competitive proposals will be received by the Mid-Florida Area Agency on Aging, Inc. until 3:00 p.m., on November 2, 2006 for the provision of an array of home and community based supportive, nutrition, disease information, health promotion and family caregiver services under the Older Americans Act to older persons residing in the following counties respectively: Alachua, Bradford, Citrus, Columbia, Dixie, Gilchrist, Hamilton, Hernando, Lafayette, Lake, Levy, Marion, Putnam, Sumter, Suwannee, and Union. The proposal package and application instructions may be obtained from the office of the Mid-Florida Area Agency on Aging, Inc. on September 29, 2006 or thereafter. A bidders conference will be conducted concerning this Request for Proposals at 2:00 p.m. on October 5, 2006.
