64B17-3.003: Licensure by Endorsement

NOTICE IS HEREBY GIVEN that on October 01, 2009, the Board of Physical therapy Practice, received a petition for waiver of the requirements of Rule 64B17-3.003, Florida Administrative Code. The Petitioner, Lakshmi I. Shankar, seeks a waiver of the requirement that applicants for endorsement meet the educational requirements contained in Rule 64B17-3.001, F.A.C.
