64B16-29.002 General Requirements.
(1) Application for an Animal Control Shelter Pharmacy permit shall be made on Board of Pharmacy approved form DOH-MQA/PH/107 “Animal Control Pharmacy Permit Application and Information,” effective October 2009, which is incorporated by reference. To obtain an application, contact the Board of Pharmacy at 4052 Bald Cypress Way, Bin #C04, Tallahassee, FL 32399-3254, or (850)488-0595, or download the application from the board’s website at http//www.doh.state.fl.us/mqa/pharmacy. The applicant shall apply to the Department of Health for Modified Class II Institutional Pharmacy Permit.
