Notice is hereby given that the following correction has been made to the proposed rule in Vol. 35 No. 50, December 18, 2009 issue of the Florida Administrative Weekly.

Rule 64B15-12.009, F.A.C., incorporated form DH-MQA 1193 into the rule. The rule did not state how the form may be obtained over the world wide web. The web address to obtain the form is www.doh.state.fl.us/mqa/osteopath/index.html.

Page one of the Application contains a typographical error. It cites “Title 42USCS subsection 666(a)(13). The citation should be to “Title 42USCA subsection 666(a)(13).”

The correction does not affect the substance of the rule as it appeared in the Florida Administrative Weekly as outlined above.
