NOTICE OF CHANGE
Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 36 No. 3, January 22, 2010 issue of the Florida Administrative Weekly.

A change was made to each of the three new forms, Notice of Change in Location of Funeral Establishment, Notice of Change in Location of Cinerator Facility, and Notice of Change in Location of Direct Disposal Establishment. The fourth sentence of the first paragraph has been changed to read “This form shall be filed with the Division at least 21 days BEFORE the new location is ready for inspection to allow the Division to schedule the inspection.” The revised forms can be found on the Division’s website at www.myfloridacfo.com/FuneralCemetery/.
