64B5-2.021: Additional Education Requirements for Reexamination

NOTICE IS HEREBY GIVEN that on April 13, 2010, the Board of Dentistry, received a petition for variance or waiver of subsection 64B5-2.021(1), F.A.C., filed by Ivette Contreras, D.D.S. Petitioner requests that the Board of Dentistry grant a waiver or variance of the requirements set forth in subsection 64B5-2.021(1), F.A.C., and allow Petitioner to sit for re-examination of the restorative part of the examination only.
