Notice is hereby given that the following changes have been made to the proposed rule in accordance with subparagraph 120.54(3)(d)1., F.S., published in Vol. 36 No. 7, February 19, 2010 issue of the Florida Administrative Weekly.

The change is in response to written comments submitted by the staff of the Joint Administrative Procedures Committee. The changes are as follows:

1. Subsection (1) shall now read as follows:

(1) All applicants for licensure as a Clinical Laboratory Personnel Director, Supervisor, Technologist, or Technician shall apply to the Department on Form #DH-MQA 3000 (04/10) “Application for Clinical Laboratory Personnel Director, Supervisor, Technologist, and Technician” which is incorporated by reference herein, copies of which can be obtained from the Board office at 4052 Bald Cypress Way, Bin #C07, Tallahassee, Florida 32399-3257, or from its website at: http://www.doh.state.fl.us/mqa/ClinLab/index.html. The application must be accompanied by the appropriate application fee required by Rule 64B3-9.001, F.A.C.
