NOTICE IS HEREBY GIVEN that on January 07, 2011, the Board of Physical Therapy Practice, received a petition for a temporary waiver filed by Louis A. Greenwald, from Rule 64B17-3.003, F.A.C., with regard to practicing physical therapy in the state of Florida.
