Documentation of Eligibility for Registration

PURPOSE:

The Board proposes this rule amendment to establish necessary documentation of eligibility for registration, delete unnecessary language and update the registration form.

This rule amendment is being proposed to establish necessary documentation of eligibility for registration and incorporates by reference the registration form.
