PURPOSE:

Rule 69L-6.012, F.A.C., amended to incorporate by reference all forms contained in Rule 69L-6.009, F.A.C., has rendered Rule 69L-6.009, F.A.C., duplicative and unnecessary. Rule 69L-6.009, F.A.C., is thereby repealed.

SUMMARY:

The amendment of Rule 69L-6.012, F.A.C., renders Rule 69L-6.009, F.A.C., duplicative and unnecessary. Rule 69L-6.009, F.A.C., is repealed.
