Board of Veterinary Medicine Departmental Forms

PURPOSE:

The purpose and effect of the proposed rule is to adopt new Veterinary Medicine application forms, DBPR VM 1-13.

SUMMARY:

Within the first paragraph the Department proposes to add language to provide the contact information for individuals who want to obtain a Veterinary Medicine application form. The following subsections provide information for a person desiring licensure and the necessary application to submit.  A subsection is provided for each type of licensure and application.
