PURPOSE:

This rulemaking is for the purpose of repealing rule 61B-15.0012, F.A.C.  All the forms listed in this rule have been incorporated in other rules under Chapter 61B-17, F.A.C., and this rule is no longer necessary.

SUMMARY:

This rule amendment repeals a rule that is no longer necessary.
