60S-4.002

Statements of Policy

PURPOSE:

To incorporate by reference two revised division forms.  The revisions to Form SAPS are to add telephone numbers, correct the PO Box and Division in the address section and the revision to both Form SAPS and Form AAPS are to remove the Social Security Number field from each since that field is no longer a required field.
