Reactivation of Inactive Licenses

PURPOSE:

This rule amendment is to revise Application for Reactivation of Inactive Psychologist Licensure.

SUMMARY:

The rule amendment will update the application form # DH-MQA 1239, “Application for Reactivation of Inactive Psychologist Licensure,” revised 4/2014.
