PURPOSE:

The Board proposes the development of several rule amendments to incorporate the revised application form, the revised protocol form and to incorporate the financial responsibility form into the appropriate anesthesia assistant rules.

SUBJECT:

Incorporation of appropriate forms into the anesthesia assistant rules.
