Application for Prescription Drug Wholesale Distributor Permit

PURPOSE:

The Division proposes to adopt and incorporate the division’s permitting application forms into rule.

SUMMARY:

The proposed rule adopts and incorporates the division’s permitting applications for entities that are required to be permitted as a prescription drug wholesale distributor.
