Application for Licensure

PURPOSE:

The Board proposes a rule amendment to revise and incorporate the application Form DH-MQA-NHA002 and the application Form DH-MQA-NHA5936, substantially modifying the current format and some language of the existing forms.

SUMMARY:

The proposed amendment updates the rule and incorporates the application Form DH-MQA-NHA002 and the application Form DH-MQA-NHA5936.
