PURPOSE:

The proposed amendment incorporates the revised license application.

SUMMARY:

The revised license application asks whether the applicant is listed on the U.S. Department of Health and Human Services Office of the Inspector General’s list of  Excluded Individuals due to a defaulted or delinquent student loan.
