64B13-4.007	Optometry Faculty Certificate

PURPOSE:

For Rule 64B13-4.004, F.A.C., the Board proposes the rule amendment to revise the incorporated application form. For Rule 64B13-4.007, F.A.C., the Board proposes the rule amendment to revise the incorporated Initial Optometry Faculty Certificate application.

SUMMARY:

The rule amendments will incorporate the revised forms.
