Office Surgery Registration Requirements, Fees

PURPOSE:

To update the form to register an office surgery to clarify the entities approved to provide the ACLS course; to add city and state to the address requirement; and, to eliminate the practitioner code.

SUMMARY:

Updates the form to register an office surgery.
