64B-7.001

Pain Management Clinic Registration Requirements; Exemption Certificates; Inspection and Registration Fees

PURPOSE:

To update the form to register a pain management clinic and to apply for an exemption from the pain management clinic registration requirement.

SUMMARY:

Updates the forms for registering and exempting a pain management clinic from registration.
