59A-11.003

Licensure Procedures

PURPOSE:

The Agency is proposing to amend Rule 59A-11.003, F.A.C., Licensure Procedures, to update incorporated application forms, remove obsolete language, and add sunset language.

SUMMARY:

The proposed changes in Rule 59A-11.003, F.A.C., will update the incorporated application form, AHCA Form 3130-3001,  strike specific licensing fee amount from rule, and remove obsolete language.
