Collaborative Practice Certification: Chronic Health Conditions

PURPOSE:

The Board proposes the rule amendment to add a chronic health condition to the list of chronic health conditions for which a pharmacist certified pursuant to Section 465.1865, F.S., can provide specified patient care services to patients of a collaborating physician pursuant to a pending Collaborative Pharmacy Practice Agreement.

SUMMARY:

A chronic health condition will be added to the list of chronic health conditions for which a pharmacist certified pursuant to Section 465.1865, F.S., can provide specified patient care services to patients of a collaborating physician pursuant to a pending Collaborative Pharmacy Practice Agreement.
