Application for Initial License

PURPOSE:

The Board proposes a rule amendment to update and make current the Hearing Aid Specialist Application for Licensure Form, Form DH-MQA 1156.

SUMMARY:

The proposed rule amendment updates and incorporates Hearing Aid Specialist Application for Licensure Form, Form DH-MQA 1156 for application for initial license.
