RULE:

64B1-4.0011

Documentation Necessary for Licensure Application

PURPOSE:

Proposed changes to the form incorporated in the rule reflect new requirements for applicants for licensure to submit to background screening requirements, in accordance with s. 456.0135, Florida Statutes.  Applicants are required to apply to the Department of Health on Form DH-MQA-1116, Application for Acupuncture License (Revised 3/25), which is incorporated by reference in rule 64B1-4.0011, F.A.C.  The new background screening requirements will go into effect beginning July 1, 2025.

SUMMARY:

Proposed changes to the form incorporated in the rule reflect new requirements for applicants for licensure to submit to background screening requirements, in accordance with s. 456.0135, Florida Statutes.
