Requirements for Renewal of Provisional License

PURPOSE:

Modifications to the rule and its incorporated Form DH-SPA-5, Application for Renewal of a Speech-Language Pathologist or Audiologist Provisional License, are being made to reflect new background screening requirements in s. 456.0135, F.S.  The modifications became effective on July 1, 2025, in accordance with Chapter 2024-243, Laws of Florida.

SUMMARY:

Modifications to the rule and its incorporated Form DH-SPA-5, Application for Renewal of a Speech-Language Pathologist or Audiologist Provisional License, are being made to reflect new background screening requirements in s. 456.0135, F.S.
