Routine Consent - Authority for Evaluation and Treatment (AET)

PURPOSE:

The amendment updates and corrects a form providing limited consent for the evaluation and treatment of detained youth.

SUMMARY:

The amendment corrects the Limited Consent for Evaluation and Treatment (Form HS 057), eliminating an unnecessary reference to vaccination and removing limiting language on the use of antihistamines.
