PURPOSE:

Revises the rule on requests for vital records to update forms and add a form to apply for a Florida marriage search and revises the rule on issuance of birth certificates used for the apostille process.

SUBJECT:

Application forms to request vital records from the Bureau of Vital Statistics and birth certificates used for the apostille process.
