Overview

RULE:

64B5-2.014	Licensure Requirements for Applicants from Accredited Schools or Colleges
64B5-2.0146	Licensure Requirements for Applicants from Non-Accredited Schools or Colleges

PURPOSE:

The Board proposes the rule amendment to update the rules incorporated Form DH-MQA 1182 – Dentistry Licensure Application to include new language regarding the submission of information reflecting adverse action taken against the applicant and published on the National Practitioner Data Bank.

SUMMARY:

To incorporate the update application form into the rules.
