Reinstatement of a Null and Void Licensee Fee

PURPOSE:

The purpose and effect of the rule is to establish the fee amount for reinstatement of a null and void license.

SUMMARY:

The rule establishes a fee of $260.00 which is to be utilized in the reinstatement of a null and void license.
