Enrollment Procedures for New Hires

PURPOSE:

To adopt a revised form and to clarify certain procedures.

SUMMARY:

To adopt the revised version of the “EZ” enrollment form; to specify what information needs to be completed on the form; and to indicate beneficiary designations may be done electronically.
