PURPOSE:

The Board proposes the rule amendment to: add the number to the form as required by Section 120.55(1)(a)4., F.S.; provide that the Licensure Verification Form be submitted to both state and countries where the applicant has held a “license” to practice respiratory Care; to delete from the form the question “Is there any derogatory information?” and replacing it with “Has any other action been taken against this applicant?” on the application.

SUMMARY:
