64B-4.003

Office Surgery Registration Requirements, Fees

PURPOSE:

To update, reorganize, and add questions to the registration applications in accordance with legislation passed during the 2009 Session.

SUMMARY:

The rule is being amended to incorporate by reference updated forms as revised in January 2010.
