RULE:

69K-1.001

List of Approved Forms; Incorporation by Reference.

PURPOSE:

Section 497.456(13), F.S., authorizes the Department to adopt rules, including: (i) forms to be used in filing claims against the Preneed Funeral Contract Consumer Protection Trust Fund (CPTF); (ii) procedures to be used for filing claims against the CPTF; (iii) information and supporting documentation that must be provided by claimants to support claims against the CPTF; (iv) procedures for the investigation of claims against the CPTF; and (v) criteria to be used in determining whether a claim is allowable and in what amount.

SUBJECT:

The proposed rule amendment adopts four new forms that shall be used to file claims against the CPTF.
