Reactivation of Inactive Licenses

PURPOSE:

The proposed rule amendment is intended to incorporate the revised application for reactivation of an inactive license and to set forth the appropriate website address for obtaining said application.

The proposed rule amendment incorporates the revised application for reactivation of an inactive license in the rule and sets forth the website address for the purpose of obtaining said application.
