PURPOSE:

The purpose of this rulemaking is to update a form reference, update materials incorporated by reference to streamline the registration or exemption process and comply with statutory changes made to the health studio registration laws by Section 25, Chapter 2017-85, Laws of Florida.

SUBJECT:

Updated FDACS-10300, Health Studio Registration Application and new FDACS-10301 Health Studio Affidavit of Exemption.
