Apprenticeship Requirements and Training Program

PURPOSE:

The Board proposes the rule amendment to update Forms DH-MQA 1180, Apprentice Optician Application, and Form DH-MQA 1063, Apprenticeship Sponsor, and to give the web site where the form can be obtained
