64B33-2.001

Licensure Requirements

PURPOSE:

The Board proposes a rule amendment to update the rule language to be consistent with statute and incorporates Form DOH-AT-001 with updated criminal and health history questions.

SUMMARY:

The proposed rule amendment updates and incorporates licensure application Form DOH-AT-001 and adds licensure requirement language providing for an alternate education pathway.
