PURPOSE:

The Board proposes the removal of a portion of the rule as the incorporated form was recently added to the application contained in subparagraph (1) of the rule.

SUMMARY:

The proposed amended rule removes subsection 2 and implements the incorporated form that was recently added to the application contained in subparagraph (1) of the rule.
