Licensure Requirements

PURPOSE:

The purpose and effect of the revision is to notify applicants of Department revisions to form DOH-AT-001 entitled “Athletic Training Application for Licensure,” (Revised 03/2024) and direct applicants to the application online.

SUMMARY:

The proposed amendment updates the revision date of form DOH-AT-001 entitled “Athletic Training Application for Licensure,” to reflect 03/2024 revisions to the application and to the applications location online.
