PURPOSE:

The Board proposes an amendment that updates the rule and makes current Application for Chiropractic Physician Initial Licensure on Form DH-MQA 1147.

The proposed amendment updates the rule and incorporates Application for Chiropractic Physician Initial Licensure on Form DH-MQA 1147.
