PURPOSE:

The proposed rule amendments are intended to incorporate the revised applications for licensure into the appropriate rules.

SUMMARY:

The proposed rule amendments incorporate the revised application forms for licensure into the appropriate licensure rules.  Specifically, the revised forms include clarifying language with regard to financial responsibility; correcting language with regard to military veterans’ fee waiver and military fee waiver; deleting questions with regard to applicant’s height, weight, etc.; and correcting footers in the applications.
